We know deciding on your health plan and other benefits is hard.  We are prepared to educate you, so you are confident in your decisions.

Bellavance Trucking is proud to offer a Medical Plan through Cigna to eligible employees.

HRA Benefit Information – Employer annual funding of HRA2 Catastrophic Coverage increases to $3,900 (individual) and $7,800 (couple or family). Employee’s shared responsibility does not change. It is still $2,000 (individual) and $4,000 (couple or family).

Dental Benefit Information – no plan changes

Vision Benefit Information – no plan changes

Voluntary:  Bellavance Trucking offers voluntary products through Renaissance (previously Security Mutual).

  • Term Life and AD&D – Current employees that waived coverage during the initial enrollment have the opportunity to enroll with evidence of insurability (medical questions). Employees currently enrolled on the plan can increase coverage up to Guarantee Issue ($200,00) for themselves and up to $25,000 for a spouse.

  • New For 2022 – Identity Theft. We are pleased to offer this valuable voluntary benefit for 2022 through ID Watchdog.
Forms and Plan Documents

Enrollment will be automatic if you are not making changes, otherwise you must access Online Enrollment:

  • Change your existing plan elections for Medical, Dental, Vision, and Voluntary Life, Short Term Disability, and Long Term Disability;
  • Elect new Accident coverage;
  • Make changes to your current dependent information and dependent coverage;
  • Elect coverage as a new participant;
  • Waive coverage.

Out of State employees will be contacted individually to make adjustments to their benefits

Medical Benefits

Eligibility

Employees who work at least thirty (30) hours per week are eligible for medical coverage 1st of the month following 60 days.

Medical Plan

Bellavance Trucking is offering all eligible employees a health plan through Cigna.

Telehealth through MDLIVE

Employees enrolled under the Cigna medical plan have access to telehealth services through MDLive.  Via phone or video conference you can consult with a board certified physician to obtain convenient care for minor conditions/symptoms. You are encouraged to register now, so you can avoid this process in the future when circumstances cause you to obtain the consult.  Any payments for these services are automatically applied to the out-of-pocket maximum under your plan.

Eligibility

Employees who work at least thirty (30) hours per week are eligible for medical coverage 1st of the month following 60 days.

HRA

Bellavance Trucking provides a Health Reimbursement Account through Healthy Dollars which works directly with your group medical policy. Bellavance will fund $1,000 for an individual contract, and $2,000 for a double/family contract. An additional $3,900 will be made available after the individual has satisfied their shared responsibility of $2,000. For double/family contracts its an additional $7,800 after they have satisfied their responsibility of $4,000 in charges.

My Vision Benefits

Eligibility

All full-time employees who work at least thirty (30) hours per week are eligible the first of the month following 60 days.

The Vision Plan monthly premium is employer paid for all employees.

My Dental Benefits

Eligibility

Employees who work at least thirty (30) hours per week are eligible for coverage on the first of the month following 60 days.  Dependent children are covered up to their 20th birthday.

Dental Plan

We provide our employees with a dental reimbursement plan.  This plan is provided to every employee at no cost once you have met your eligibility period.

Contributions

The Dental Reimbursement plan is employer paid for all employees.

Voluntary Life Benefits

Eligibility

Employees who work at least thirty (30) hours per week are eligible for coverage on the first of the month following 60 days.

Life Insurance Contributions
This benefit is 100% employee paid. Premiums will be deducted on weekly basis.
*Rates are per $1,000 of benefit for employee and spouse.

**Rates are $0.490 per $1,000 benefit for children.

Voluntary Short and Long Term Disability Benefits

Eligibility

All full-time employees who work at least thirty (30) hours per week are eligible for coverage the first of the month following their date of hire.

Short Term Disability Contributions
This benefit is 100% employee paid. Premiums will be deducted on weekly basis.
*Rates are per $10 of weekly benefit.
Long Term Disability Contributions
This benefit is 100% employee paid. Premiums will be deducted on weekly basis.
*Rates are per $100 of weekly benefit.

Eligibility

In order to participate in the plan, you must satisfy certain age and service conditions under the plan:
